Abstract

Pulmonary tuberculosis (TB) is a contagious infectious disease that remains a significant public health burden, particularly in developing countries such as Indonesia. In addition to primary infection by Mycobacterium tuberculosis, TB patients are also at risk of secondary infections caused by opportunistic bacteria such as Escherichia coli, Enterobacter cloacae, Haemophilus influenzae, and Pseudomonas aeruginosa. These secondary infections may exacerbate clinical symptoms and prolong the treatment process. This study aimed to detect the presence of the four bacteria in saliva samples from pulmonary TB patients using the Multiplex RT-PCR method. This technique allows for the simultaneous detection of multiple genetic targets in a single PCR reaction, with high sensitivity and specificity. Saliva was chosen as a sample due to its non-invasive collection method, ease of access, and its ability to represent the flora of the upper respiratory tract. The study was conducted in May 2025 at the Center for Diagnostic and Infectious Disease Research Laboratory, Faculty of Medicine, Andalas University, using four saliva samples from confirmed TB patients. The analysis included DNA isolation, purity measurement, and bacterial detection using the Crown Lab® Pneumoplex 3.0 kit. Results showed that H. influenzae was detected in all samples, while E. coli appeared in only one sample with a high Ct value. E. cloacae and P. aeruginosa were not detected. In conclusion, Multiplex RT-PCR is effective for detecting secondary bacterial infections, and saliva is a valid sample for molecular examination Keyword: Pulmonary tuberculosis, secondary infection, saliva, Multiplex RT-PCR.
